Relieving Your Pain, Stress, & Anxiety

 

The palliative care team at Signal Hill helps you recover from your disease by managing symptoms as you go through often challenging medical treatments or procedures, like surgery or chemotherapy, such as pain, anxiety, nausea, or lack of appetite. Our social workers offer assistance, direction, and coordination for beneficial services, such as food preparation, personal care, and other supports for daily living activities.

Getting to Know Your Healthcare Professionals
 

Our team collaborates with your doctor's treatment plan; we do not take the place of it. Our experts with board certification in palliative care offer an additional level of support. They look after you while other medical professionals try to treat your illness. In other words, while you and your physicians search for a cure, we assist in managing your symptoms and discomforts.

The palliative care team will take the time to explain your condition and give you access to services and information that will make managing it easier for you. We'll address your concerns regarding the numerous treatment options at your disposal and how they can impact your quality of life. Consultations that Suit You
 

Our board-certified physicians and palliative-trained nurse practitioners offer consultations at your home, a senior living facility, an outpatient office, specific hospitals, or via telehealth in collaboration with other doctors on your healthcare team.

Assistance with Advance Directives

​

As professionals, we may also aid in the completion of advance directives like the Physician's Order for Life-Sustaining Treatment (POLST), which specifies the medical care you would or would not want if you were unable to communicate your wishes. Making healthcare decisions when there is no verbal or written plan in place can be difficult for family members. By completing the POLST form, you are providing family members with a roadmap to help them carry out your wishes.

Navigating Complex Medical Decisions

​

Palliative care provides assistance in evaluating your medical needs and making healthcare decisions. There may be circumstances that are not specifically covered by a POLST form or other advance directive, or a family member may feel conflicted when making a decision.
